We collect the information on this form to ensure that your child's participation in Goodlife activities is as enjoyable and safe as possible. Failure to supply the information will prevent your child's participation in Goodlife Kids Holiday Program. Please advise us if any of the following details in this form change.
Personal information collected by us is only used or disclosed for the purpose of running Goodlife activities, to provide emergency health care and recording incidents involving your child.
IMPORTANT
Once your child has been signed in, it is important that they stay with the group. Running around the centre, cafe or grounds without a leader's knowledge makes it very hard to safely care for your child. Please ensure that your child/ren understand this.
This program is for children in grades 1 to 6 ( 2025).
I /We understand that, whilst Goodlife Community Centre will take reasonable steps to provide a safe environment, accidents do occur. I/We acknowledge that Goodlife Community Centre will not be liable for any injury that may occur, which arises either directly or indirectly from, or in connection with, Goodlife Kids activities. I /We agree that Goodlife Community Centre may authorise whatever medical treatment is required (including ambulance attendance/transport and hospital treatment. I/We agree to pay all medical expenses.
By completing this form, I the carer of the child listed, acknowledge that I have read the pool rules (below) and agree to comply with the rules that have been set out to ensure the safety of my child. I understand that failure to abide by these rules, may result in my child being removed from the pool.
Pool Rules
- Only enter when there is a Lifeguard on duty
- No diving or bombing
- No ceramic cups or drinking glasses in the pool area
- No running
I release indemnity and hold harmless the Goodlife Community Centre operator, its servants and agents, from and against all and any actions that may be made by me or on my behalf, or by other parties for or in respect of, or arising out of any injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any way whatsoever. I also agree that in the event that I am injured or my property is damaged, I will bring no claim, legal or otherwise, against the Goodlife Community Centre operator in respect to that injury or damage.
I/We recognise that being part of a community involves mutual care and consideration and therefore agree that unacceptable behaviour may result in our/my child/ren being sent home and/or being temporarily or permanently prohibited from attending the Kids activities.
I / we will remain contactible for the duration of the program and will collect my child/ ren if required.
