I /We understand that, whilst Hot Chocolate Circle and Goodlife Community Centre will take reasonable steps to provide a safe environment, accidents do occur. I/We acknowledge that Goodlife Community Centre will not be liable for any injury that may occur, which arises wither directly or indirectly from, or in connection with, Hot Chocolate Circle activities. I /We agree that Goodlife Community Centre may authorise whatever medical treatment is required (including ambulance attendance/transport and hospital treatment. I/We agree to pay all medical expenses.

I/We hereby agree to indemnify and hold harmless Hot Chocolate Circle and Goodlife Community Centre against any and all claims, demands, suits and liability of whatever nature and howsoever arising from, or in connection with, any injury that may be suffered to my child, or that my child may cause to another person, as well as any loss or damage to property, equipment or personal effects belonging to my child, or any other person, arising either directly or indirectly out of, or in connection with Hot Chocoloate Circle.

I/We recognise that being part of a community involves mutual care and consideration and therefore agree that unacceptable behaviour may result in our/my child/ren being sent home.
