Goodlife Youth is about building community for high school students (grades 7-12). It is expected that youth and leaders contribute to this through respect for one another and their character. Behaviour which undermines our commitment to grow community through respect for one another and the character of our youth will not be tolerated. We collect the information on this form to ensure that your child's participation in Goodlife activities is as enjoyable and safe as possible.  Personal information collected by us is only used or disclosed for the purpose of running Goodlife activities, to provide health care and recording incidents involving your children.  You can unsubscribe from group communication at any time.

I understand that unacceptable behaviour may result in me/my child* being sent home or being disallowed from attending Goodlife Youth for a time.

I acknowledge that Goodlife Youth and Goodlife Community Centre will take reasonable steps to provide a safe environment, accidents do occur. I acknowledge that Goodlife Community Centre will not be liable for any injury that may occur, which arises either directly or indirectly from, or in connection with, Goodlife Youth activities.

I agree that Goodlife Community Centre may authorise whatever medical treatment is required (including ambulance attendance/transport and hospital treatment). I agree to pay all medical expenses incurred.

I hereby agree to indemnify and hold harmless Goodlife Youth and Goodlife Community Centre against any and all claims, demands, suits and liability of whatever nature and howsoever arising from, or in connection with any injury that may be suffered to me/my child*, or that I/my child* may cause to another person, as well as any loss or damage to property, equipment or personal effects belonging to me/my child*, or any other person, arising either directly or indirectly out of, or in connection with Goodlife Youth activities.
