We collect the information on this form to ensure that your child's participation in Goodlife activities is as enjoyable and safe as possible. Failure to supply the information will prevent your child's participation in Goodlife Kids Friday Boost activities. Please advise us if any of the following details in this form change.

Personal information collected by us is only used or disclosed for the purpose of running Goodlife activities, to provide emergency health care and recording incidents involving your child.

IMPORTANT

Once your child has been signed in, it is important that they stay with the group. Running around the centre, cafe or grounds without a leaders knowledge makes it very hard to safely care for your child. Please ensure that your child/ren understand this.

BOOST Kids is for children in grades 1 to 6. The only exception to this is if you participate as a leader and understand that you take full responsibility for your own children.

I /We understand that, whilst BOOST Kids and Goodlife Community Centre will take reasonable steps to provide a safe environment, accidents do occur. I/We acknowledge that Goodlife Community Centre will not be liable for any injury that may occur, which arises wither directly or indirectly from, or in connection with, BOOST Kids activities. I /We agree that Goodlife Community Centre may authorise whatever medical treatment is required (including ambulance attendance/transport and hospital treatment. I/We agree to pay all medical expenses.

I/We hereby agree to indemnify and hold harmless BOOST Kids and Goodlife Community Centre against any and all claims, demands, suits and liability of whatever nature and howsoever arising from, or in connection with, any injurey that may be suffered to my child, or that my child may cause to another person, as well as any loss or damage to property, equipment or personal effects belonging to my child, or any other person, arising either directly or indirectly out of, or in connection with BOOST Kids.

I/We recognise that being part of a community involves mutual care and consideration and therefore agree that unacceptable behaviour may result in our/my child/ren being sent home and/or being temporarily or permanently prohibited from attending BOOST Kids activities.
